Guide to Fill and Submit the Admission Form

  1. Course and Personal Details: Fill in the name of your course and your personal information, including your name (in BLOCK LETTERS), date of birth, gender, blood group, and contact details (mobile number and email).
  2. Parent/Guardian Details: Provide your parents’ names, occupations, qualifications, income, and contact numbers.
  3. Address: Enter both your postal and permanent addresses, including district, city, pin code, and state.
  4. Academic Information: List your 10th, 10+2, and any higher education details (e.g., graduation), including school/college name, board, year of passing, subjects, and marks. Attach photocopies of your certificates.
  5. Hostel Requirement and Exam Language: Specify if you need hostel accommodation and your preferred exam language (Hindi or English).
  6. Declaration: Read the declaration carefully, sign, and date the form. Your parent/guardian must also sign the undertaking section.

Submission

  • Attach all required documents, including your mark sheets, a self-attested passport photo, and a copy of your Aadhar card.
  • Submit the completed form in hard copy at the college admission office during admission. Bring original documents for verification.
